Add custom fields in the module for feedback submission
I'd like to be able to add a field that allows users to optionally enter their email address. Is this possible?

If we correctly understand your case, you want to allow users to leave contact.
So you can follow up with them.
We collaborate now and improve feedback form to allow users do this, will try to do asap this feature, will provide screenshots of the new form soon.
this feature would be awesome if the info could be saved with the comment and viewable only to admins

Custom fields are now available in the topic and user profile, So, now it is possible to create some additional fields like email, etc.
